Because it was categorized as an additional tank picture, rather than a main tank picture. Reupload it as a main pic, or ask the webmaster to take care of it.
It can also take some time beyond approval unless I just have a slow machine. I think teh last time I updated my main profile picture it took like an additional 24-48 hrs after the photo was approved for it to show up as my little 'avatar'
